PCN/HR Manager

The PCN Manager supports the delivery of the Primary Care Network agenda. They will support the PCN on a day to day basis in all elements of governance and management and to support in the delivery of high quality primary health care with the practices in the PCN.

PCN Care Co-ordinators

Help, manage patient care. Co-ordinate and monitor patient treatment plans and signpost to relevant healthcare professional. They will work closely with the GPs and other primary care professionals within the PCN to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed.
